Your completed form and/or resume, when received, will be copied and distributed to the members of our Search Committee. To ensure legibility of the copies, please type or print clearly using black ink. No information provided will go beyond our Search Committee & District Superintendent without your permission. Please attach a recent photograph of yourself if one is available.

Please provide, on an attached separate sheet of paper, the names and contact information of five people who know you well and would be willing to serve as references. One of these should be your present District Superintendent or the District Superintendent for the last church in which you served. A second should be a staff person who has worked under your supervision and mentoring. The other three should be church lay leaders you have pastored. Please let each of these people know that we may be contacting them in the next few weeks or months.

Personal Christian Testimony

Please provide, as an attachment, your personal Christian testimony identifying and describing those experiences that you view as being turning points in your life and in the development of your Christian faith and pastoral ministry.

Pastoral Experience

Please briefly discuss, in an attachment, the hi-lights of your previous pastoral experience. For each church you have pastored, discuss what your responsibilities were, how long you served, the most important contributions you believe you made, and your reasons for leaving. If you have had a relatively long pastoral career, focus on your last two or three pastorates.

Thank you for your time and effort in providing the information requested in this Pastor’s Information Form. In addition to your resume and /or this completed form, please provide two recordings (video preferred) of typical sermons you have preached in the past six months and sign the following release:

Please Note: All candidate applications must be reviewed and approved by the Northeastern C&MA District Superintendent for our Search Committee’s consideration. Therefore, two sets of copies of all materials must be provided: 1) one sent to our Search Committee at the address given below, and; 2) a second sent to our District Superintendent, Rev. Dan Miller, whose contact information is provided below.
